About JotForm.
JotForm is a general-purpose online form builder headquartered in San Francisco, CA. It is not an ABA-specific data-collection system.
How JotForm works.
JotForm is a web-based form builder and no-code platform founded in 2006 by Aytekin Tank. Headquartered in San Francisco, California, the company provides tools that allow users to create custom online forms, surveys, and data collection workflows without requiring coding knowledge.
While JotForm offers ABA Assessment Form templates and Applied Behavior Analysis Service Request Form templates, the platform itself is a generic form-building tool rather than a specialized clinical ABA software solution. ABA providers can use JotForm to collect intake information and service requests, but the platform does not include dedicated ABA-specific features such as automated progress graphing, behavioral tracking, or clinical workflow management.
The platform is trusted by over 35 million users worldwide and serves diverse industries including healthcare, education, e-commerce, nonprofits, government, and small to mid-sized enterprises. JotForm competes in the broader low-code/no-code application development space alongside platforms like Formstack, Typeform, and Microsoft Power Apps.
As a private company with approximately 900 employees, JotForm has maintained bootstrap-style growth and remains actively developed. The company positions itself as a flexible platform capable of supporting everything from simple contact forms to complex enterprise workflows, with integrations to hundreds of third-party business applications.

I had an unauthorized charged from jotform, they have no support so it's impossible to reach someone if you don't have an account. I submitted a ticket and everytime they tell me it's been refunded but it was never refunded. Not a trustworthy company, and horrible support since you can't reach anyone.

If I could give ZERO stars I would. Absolutely horrific system. They offer AI to "write" your app but the AI doesn't follow directions. I took the time to figure out how/what elements to add or remove to fix the mess up. I signed up as a test user (by the time I was done a dozen times) and it took most of those before it would recognize me as a user at all. The app also shows the data one user puts in to all the other users (I created multiple test users). Then when clicking on features in the app created it would show 404 error and a ton more issues. The one thing I liked is that a "live" person took over from the AI help when it was clear we were going in circles. I was following the directions and it still wasn't working. However, the live help person just kept trying to point to browser or user error instead of recognizing it was 100% platform failure. I use technology all day every day and dozens of programs. This was by far the WORST experience I've had in a long time. I can't trust the platform because it doesn't keep individual user data private it shares it with every other user of the created app. Despite having the setting done correctly by their own instructions. In addition the live representative couldn't answer and ignored most of my questions since there was such a huge list of problems. I would NOT use them again. This is my opinion. I was creating a simple app with simple functions and it was a nightmare.

ZERO support. Custom CSS option doesn't work. Their AI chat bot will lie to you about information and waste your time. EVEN their support tickets are handled by AI! What a waste of money

Be forewarned that Jotform auto-renews your biannual or annual plan at whatever the going rate is without invoicing you first, in this case a ridiculous $816 for their lowest tier paid plan, Bronze, which they deducted from my bank account without a prior invoice. I immediately, as in minutes, upgraded to an on-sale year-end higher plan. But instead of refunding my bank account the over $500 difference, they instead left it indefinitely on my JotForm account. (hoping I wouldn't notice?) The kicker was that after their AI chat-bot finally referred me to their human-chat customer service team (no phone support available), they refused to refund my bank account until I filled out a "refund form," which their "relevant team" would then review, and in 10 business days refund my bank account. After 6 years with them, and multiple referrals by me to them, I would expect a little empathy and better customer support. Update: It's day 3, and countless hours later of dealing with Jotform's so-called customer service team, and Jotform continues to refuse to refund my money, stating repeatedly their patent company excuse of referring my requests to their (fictional?) "relevant team," along with ignoring my requests to be escalated to a supervisor. I'd had enough yesterday and asked to be downgraded to their free Starter plan and ALL of my $816 refunded (twenty-nine days before their 30-day money back guarantee expired), which they ignored like I had never asked. I've filled out their requested "refund form" twice, which their "relevant team" is supposed to review and respond to, and they don't acknowledge it.

What can I say about Jotform and my experience with the platform thus far? I have been on the platform for nearly a month now and have discovered that without this program, I would be lost in tracking all the information I must track for a special custom corporate order. I am not a computer guy, I am not a numbers guy, and I am surely not a tracking guy. My skills fall into a much different category, but when it was time to get a custom corporate timepiece, I was going to be the guy. Watches are one of my greatest passions, and there was no way I was going to allow somebody else to get to head the project. The downside was that I would also be responsible for disseminating all the information to and tracking it from over 30 subdivisions of a 10,000+ employee company that is spread across approximately 500 square miles. How on earth was I going to meet this requirement? I got to work and solicited the advice of those who had traveled a similar road before me. None of them led me to Jotform, though. This little gem I stumbled across, all on my own, but we will get to that in a second. I had seen QR codes used to share and spread information far and wide. I immediately found a platform where I could create a QR code and bend it to my will. Once I established the QR code, I set to work... ...I had to find a way to allow people to register, and then have all the information collected in the registration process so that it could be neatly organized. It also needed to be managed in a way so that I could read it and understand it. I also needed the ability to check the analytics and see where I was missing bigger portions of my targeted audience. There were aspects that I did not even know I would need until I found Jotform, and what a lifesaver it was that I came across it. Jotform has made it absolutely simple to create a registration document that my co-workers can locate through the third-party QR code. If interested in the timepiece, they just fill out the information that I set up on the Jotform document page and then hit submit. Once all the information is tracked on Jotform's spreadsheet, I am immediately notified via email, and I can go in and review the submission. I am also able to edit and amend submitted information as needed. I would truly still be lost in my Microsoft Excel worksheets today if Jotform did not exist. I have over 20 points of contact across my company; each point of contact is responsible for their subdivision within the company, and because of Jotform, they do not have to personally obtain the needed information for registration and then send it up to me for data entry and tracking. Instead, I just send the points of contact the automatically updated spreadsheet twice a week so that they can track their own people and keep them up to date on the latest information put out by the manufacturer of our corporate timepiece. I honestly could not accomplish what I have been able to accomplish without Jotform. This process has freed up time that I once had to spend on trying to track and reconcile names, personal information, and assignments throughout the company, and then subsequently put into a Microsoft Excel worksheet and then try and share with the points of contact. Thank you, Jotform, for making this three-month process way easier than it could have or should have been! Robert in Los Angeles
